Our Live Well Program is designed for independent living residents who wish to get additional, non-clinical support for any duration at a time. This program is an “add on” to the Independent Living list of included services with a contract that is executed through Cornerstone’s Home Health entity.

MEDICATION REMINDERS
Team members assist residents with simple reminders to take their medications.

MEALS, LAUNDRY AND HOUSEKEEPING
Residents enjoy one meal per day in Independent Living and will be able to get two additional meals in this program. If the prefer, residents can certainly enjoy their meal in their home with our free delivery service or they can dine in the dining room. Residents will also enjoy a snack package that will be delivered each day with a variety of healthy snacks. Our team will do our best to include their favorite items and customize this package to their liking. Just like in Assisted Living, residents can put their items in the hallway each week to be laundered. Residents participating in the program will also receive two additional housekeeping visits with lite cleaning to supplement needs throughout the week. Trash removal, vacuuming, lite dusting and wiping down surfaces are all included.

8 HOURS OF CUSTOM SERVICES
In addition, residents will have the opportunity to customize the services they receive by selecting from a list of concierge services. These services may include things like meal prep, bed making, coordination of appointments, ironing/ folding, and putting laundry away, companionship, customized cleaning and organizing, pet support, and onsite escort services.

REGULAR WELLNESS ASSESSMENTS

REGULAR WELLNESS VISITS
Residents will have regular wellness checks made by the Social Worker, Chaplain and Life Enrichment team. Chaplain visits may consist of activities like prayer, scripture reading or faith-based conversations. Social worker visits may consist of conversations or other activities that support emotional wellness. Life Enrichment visits may consist of one-on-one visits that support social, occupational, physical, and intellectual wellness. This could be reading a book aloud, exercising, listening to music, doing a craft together, or simply having a conversation.
